New project in Abbotsford addresses seniors’ health and planning

Initiative headed by Division of Family Practice includes website and magazine

The Abbotsford Division of Family Practice, the City of Abbotsford, and multiple partners have launched a new initiative for seniors called Aging Well, Planning Well.

The project is a magazine and a website with “made in Abbotsford” articles and videos on health, finances, life planning and healthy living tips.

Hal Johnson and Joanne McLeod from Canada’s BodyBreak contributed an article on the importance of exercise and a video with fun exercise tips that one can do indoors, made especially for this initiative.

The website agewellplanwell.ca showcases videos made from local professionals including family doctors, a pharmacist, a dietitian, and local community and health leaders.

The Aging Well, Planning Well initiative is part of the Abbotsford Division of Family Practice’s Older Adults with Complex Conditions Shared Care project.

The project has multiple partners and supports doctors, other health professionals, and community leaders to better co-ordinate care for older adults (60 years and older). The project is focused on advance-care planning and the prevention and management of chronic health conditions.
